I downloaded the LIF extractor and have been having fun editing the face, torso, and leg decals, but I wonder...there are no headgear decals, so how exactly would I save them to the db folder to use in Digital Designer?

How to change:

Edit DecorationMapping.xml

Add this line:

<Mapping decorationID="The Filename" designID="3814" surfaceID="1"/>

Also, ony pieces with UV maps defined in them can be textured.

Anyway I can add UV maps to pieces? I really want to take a big plate and make it a bilboard.

Short answer: No.

Long answer: If you knew anything about 3D, you would know that UV Maps have to applied to the 3D model (in LDD's case, a .g file). In order to add a UV map to a LDD brick, one must import the brick into a 3D app (Maya, Blender, etc.), create a UV, arrange it, and out it back into LDD. We are currently unable to even do the first step (import brick). Until we can, no new UVs.
